Weak irritants may require repeated exposure to cause the problem whereas highly irritating materials may trigger hand eczema with the first exposure.

Irritants or allergens can cause disturbance of stratum corneum, the superficial layer of the skin made of dead, flat skin cells that shed about every 2 weeks. The cells of the stratum corneum are embedded in a lipid layer of ceramides, fatty acids, and cholesterol with 25% water content. The stratum corneum serves as an important barrier by keeping molecules from going into and out of the skin, thus protecting the deep layer of the skin against the exterior environment and preventing water loss.

Irritants or allergens provoke disturbance of stratum corneum by direct cell toxicity, lipid barrier removal, cell membrane damage, and destruction of keratins (the key structural material making up the outer layer of the human skin). All forms of hand dermatitis include a disruption in the stratum corneum.

In addition to irritants and allergens, some host factors may weaken the skin function and lower the repair capacity of the skin, and thus contribute to the development of hand dermatitis. These conditions include predisposition to allergy and other skin diseases for instance atopic dermatitis, psoriasis, etc.

Cellular Damage Sets Off Inflammation

Despite varying irritants or allergens, the final common path of hand eczema is implemented with the release of inflammatory mediators from keratinocytes, the major cell type in the epidermis (the top layer of the skin).

Keratinocytes are critical in the producing and releasing inflammatory cytokines upon entering of irritants through the stratum corneum. These inflammatory cytokines augment immune and allergic responses and propagate hand eczema.

Persistent Inflammation Impairs Skin Barrier Function

Hand dermatitis is often a recurrent condition owing to sustained inflammatory responses. Studies have shown that frequent contact of irritants or allergens promotes continued inflammation which exposes the immune system to immunogenic peptides and result in persistent recruitment of inflammatory cells and sustained production of inflammatory mediators. Extra virgin coconut oil has helped many I know to help heal the skin. Ultimately, of course, the immune system and body do the healing if you were to do nothing, but coconut oil can speed up the process.

You want to make sure you get the extra virgin type that is not refined, bleached or deodorized. You can order it online.

Whole food-type markets are carrying more coconut oil, in pint and quart jars and especially the extra virgin coconut oil. Get the organic kind if you can.

Coconut oil cures many other skin conditions and skin disorders too, besides making the skin soft and smooth. I use it for everything including as a make-up remover and under my foundation. My skin is always soft, silky, smooth, and free of rashes, bumps, blemishes, blisters, etc.

Coconut oil is very inexpensive therefore, a good, cheap home remedy. Keep it around to treat any other skin problem. It’s actually healthy to eat too. It’s a plant saturated fat, not an animal saturated fat.

I add two big tablespoons to my smoothies, to help my skin from the inside out too, and have been doing so for many years.

Although we refer to it as coconut oil, it isn’t actually oil until the temperature climbs above 77 degrees. It can be applied when it is in the oil-state or non-oil state.

To apply the extra virgin coconut oil, massage it deep into the affected area. You can heat it if you like but you don’t have to. Depending on how severe the eczema is, apply it three or four times a day or more and especially at bedtime.

It’s important to keep the area moist with the oil. You may have to apply a loose bandage to your skin, soaked with the oil. If the eczema is on your hands, you’ll have to apply it even more often.

Also you can expose the area to direct sunlight for 20 minutes or so each day. You’ll get your daily dose of vitamin D that way too.

Many have used coconut oil as a treatment to cure or heal their eczema or their baby’s eczema.

It just takes motivation, a willingness to try it, and diligent application of coconut oil several times each day and at bedtime.

Melatonin Might Help Sleepless Kids with Eczema

TUESDAY, Nov. 24, 2015 (HealthDay News) — Children with the skin condition eczema often have trouble sleeping. Now, a new study suggests that over-the-counter melatonin might boost their shuteye.

Eczema, also called atopic dermatitis, is characterized by an itchy, red rash. It affects as many as 30 percent of all kids, more than half of whom experience sleep difficulties, the researchers said.

These sleep problems can be difficult to treat in these children, said Dr. Yung-Sen Chang, an attending physician in pediatrics at Taipei City Hospital Renai Branch in Taiwan. Antihistamines can stop working after a few days, and tranquilizers have potentially serious side effects, Chang said.

But supplementation with melatonin, his study found, “is safe and effective for helping children with atopic dermatitis fall asleep faster.”

The link between the skin condition and insufficient sleep “has an impact on people with eczema at all ages,” said Dr. Lawrence Eichenfield, chief of pediatric and adolescent dermatology at the University of California, San Diego and Rady Children’s Hospital.

According to Eichenfield, it’s generally established that itching keeps people with eczema from getting enough sleep. However, Chang said research has questioned that idea.

In a previous study, Chang and colleagues found low levels of nocturnal melatonin in patients with eczema who had sleep problems. Those findings inspired the new study.

“Melatonin is a natural human hormone with minimal adverse effects,” Chang said, “so it seemed like a good choice for children.”

The study involved 48 children, about 22 months to 18 years old, who had eczema. The children received treatment with either an inactive placebo or a 3-milligram daily dose of melatonin at bedtime for four weeks. Thirty-eight participants then took the alternate treatment (melatonin or placebo) for another four weeks.

When the children took melatonin, the severity of eczema dipped slightly, possibly because melatonin’s anti-inflammatory effect improved the skin condition, Chang said.

Also, kids taking melatonin fell asleep about 21 minutes sooner than kids taking the placebo, the findings showed.

Total nightly sleep rose by 10 minutes on average (from 380 to 390 minutes, or 6.5 hours total) in the melatonin group, while it fell by 20 minutes among those who took a placebo, according to the report.

The study participants didn’t report any side effects. Melatonin supplements are inexpensive in the United States — under 9 cents a pill at one major supplier.

Eichenfield, who wasn’t involved in the research, said the study appears to be well-designed. Melatonin hasn’t been studied much as a sleep treatment for kids, he cautioned.

Should parents of kids with eczema and sleep problems try melatonin?

Eichenfield said melatonin might be helpful, but “we have a broad set of tools” to treat eczema and minimize its effects on children. He suggested tackling the skin condition first to try to mitigate the sleep issues.

Chang recommended that parents talk to their child’s doctor before starting melatonin. As for adults, melatonin may help them, too. But more studies are needed, Chang said.

The study is published in the Nov. 16 online edition of the journal JAMA Pediatrics.
