Melatonin Might Help Sleepless Kids with Eczema

TUESDAY, Nov. 24, 2015 (HealthDay News) — Children with the skin condition eczema often have trouble sleeping. Now, a new study suggests that over-the-counter melatonin might boost their shuteye.

Eczema, also called atopic dermatitis, is characterized by an itchy, red rash. It affects as many as 30 percent of all kids, more than half of whom experience sleep difficulties, the researchers said.

These sleep problems can be difficult to treat in these children, said Dr. Yung-Sen Chang, an attending physician in pediatrics at Taipei City Hospital Renai Branch in Taiwan. Antihistamines can stop working after a few days, and tranquilizers have potentially serious side effects, Chang said.

But supplementation with melatonin, his study found, “is safe and effective for helping children with atopic dermatitis fall asleep faster.”

The link between the skin condition and insufficient sleep “has an impact on people with eczema at all ages,” said Dr. Lawrence Eichenfield, chief of pediatric and adolescent dermatology at the University of California, San Diego and Rady Children’s Hospital.

According to Eichenfield, it’s generally established that itching keeps people with eczema from getting enough sleep. However, Chang said research has questioned that idea.

In a previous study, Chang and colleagues found low levels of nocturnal melatonin in patients with eczema who had sleep problems. Those findings inspired the new study.

“Melatonin is a natural human hormone with minimal adverse effects,” Chang said, “so it seemed like a good choice for children.”

The study involved 48 children, about 22 months to 18 years old, who had eczema. The children received treatment with either an inactive placebo or a 3-milligram daily dose of melatonin at bedtime for four weeks. Thirty-eight participants then took the alternate treatment (melatonin or placebo) for another four weeks.

When the children took melatonin, the severity of eczema dipped slightly, possibly because melatonin’s anti-inflammatory effect improved the skin condition, Chang said.

Also, kids taking melatonin fell asleep about 21 minutes sooner than kids taking the placebo, the findings showed.

Total nightly sleep rose by 10 minutes on average (from 380 to 390 minutes, or 6.5 hours total) in the melatonin group, while it fell by 20 minutes among those who took a placebo, according to the report.

The study participants didn’t report any side effects. Melatonin supplements are inexpensive in the United States — under 9 cents a pill at one major supplier.

Eichenfield, who wasn’t involved in the research, said the study appears to be well-designed. Melatonin hasn’t been studied much as a sleep treatment for kids, he cautioned.

Should parents of kids with eczema and sleep problems try melatonin?

Eichenfield said melatonin might be helpful, but “we have a broad set of tools” to treat eczema and minimize its effects on children. He suggested tackling the skin condition first to try to mitigate the sleep issues.

Chang recommended that parents talk to their child’s doctor before starting melatonin. As for adults, melatonin may help them, too. But more studies are needed, Chang said.

Making Headway Toward Causes of Eczema

MONDAY, Oct. 19, 2015 (HealthDay News) -- New gene variants associated with the skin condition eczema have been identified by an international team of researchers.

Eczema, characterized by itchy, red rashes, is known to run in families. The new findings add to the number of genetic variants known to increase risk for the condition, making the total 31.

The researchers did this by analyzing the genomes (genetic makeup) of 377,000 people worldwide.

"Though the genetic variants identified in this current study represent only a small proportion of the risk for developing eczema ... they do give new insights into important disease mechanisms," said study leader Lavinia Paternoster, an epidemiologist at the University of Bristol in England.

"Through ongoing research in this area, these findings could be turned into treatments of the future," she said in a university news release.

All of the newly identified genetic changes play a role in immune system regulation and offer potential targets for scientists exploring new treatments for eczema, the study authors said.

The team of investigators also found evidence of some genetic overlap between eczema and other conditions such as inflammatory bowel disease. This suggests that studying these diseases together could improve understanding of the disorders and possibly lead to new treatments, the researchers said.

The study was published Oct. 19 in the journal Nature Genetics.

Study co-author Dr. Sara Brown, an academic dermatologist at the University of Dundee in Scotland, said, "The very large numbers of participants in this research has allowed us to "fine-tune" our understanding of eczema genetic risk, providing more detail on how the skin immune system can go wrong in eczema."

Genetics Home Reference: Atopic dermatitis

The genetics of atopic dermatitis are not completely understood. Studies suggest that several genes can be involved in development of the condition. The strongest association is with the FLG gene, which is mutated in 20 to 30 percent of people with atopic dermatitis compared with 8 to 10 percent of the general population without atopic dermatitis.

The FLG gene provides instructions for making a protein called profilaggrin, which is cut (cleaved) to produce multiple copies of the filaggrin protein. Filaggrin is involved in creating the structure of the outermost layer of skin, creating a strong barrier to keep in water and keep out foreign substances, including toxins, bacteria, and substances that can cause allergic reactions (allergens), such as pollen and dust. Further processing of the filaggrin protein produces other molecules that are part of the skin"s "natural moisturizing factor," which helps maintain hydration of the outermost layer of skin.

Mutations in the FLG gene lead to production of an abnormally short profilaggrin molecule that cannot be cleaved to produce filaggrin proteins. The resulting shortage of filaggrin can impair the barrier function of the skin. In addition, a lack of natural moisturizing factor allows excess water loss through the skin, which can lead to dry skin.

Research shows that impairment of the skin"s barrier function contributes to development of allergic disorders. An allergic reaction occurs when the body mistakenly recognizes a harmless substance, such as pollen, as a danger and stimulates an immune response to it. Research suggests that without a properly functioning barrier, allergens are able to get into the body through the skin. For unknown reasons, in susceptible individuals the body reacts as if the allergen is harmful and produces immune proteins called IgE antibodies specific to the allergen. Upon later encounters with the allergen, IgE antibodies recognize it, which stimulates an immune response, causing the symptoms of allergies, such as itchy, watery eyes or breathing difficulty. Although atopic dermatitis is not initially caused by an allergic reaction, flare-ups of the rashes can be triggered by allergens. The impaired barrier function caused by FLG gene mutations also contributes to the increased risk of asthma and other allergic disorders in people with atopic dermatitis.

Mutations in many other genes, most of which have not been identified, are likely associated with development of atopic dermatitis. Researchers suspect these genes are involved in the skin"s barrier function or in the function of the immune system. However, not everyone with a mutation in FLG or another associated gene develops atopic dermatitis; exposure to certain environmental factors also contributes to the development of the disorder. Studies suggest that these exposures trigger epigenetic changes to the DNA. Epigenetic changes modify DNA without changing the DNA sequence. They can affect gene activity and regulate the production of proteins, which may influence the development of allergies in susceptible individuals.
